Health Interview Screening Survey

Thank you for your interest in participating in our study. This study is being conducted by RTI International (www.rti.org), a nonprofit research organization.

RTI International is currently working with the Carolina Population Center at the University of North Carolina on a survey about adolescent to adult health. We are interested in meeting with individuals who can help us review and improve the survey questions we have developed for this study. Our goal is to learn whether the survey questions are easy to understand and can be answered by most people.

The purpose of this online form is to determine if you are eligible to participate in a private, online video interview to test the questions in our survey. This online form will ask questions about your background. You may find some of these questions to be personal and you can stop at any time.

If you are eligible and we select you for an interview, you will be called by a researcher to schedule an appointment for a 90-minute video interview conducted online through Zoom. Participants must use a desktop, laptop, or tablet computer (not a mobile device) that is connected to the Internet to complete the interview. If you complete the interview you will receive a $60 electronic gift card via email to thank you for your time. RTI International employees and their family members are not eligible to participate.
